But all the horn contact does is ground the horn. Power runs straight to a fuse that runs to battery power. If there is no ground, it won't sound.. So then how does one do it?

One side of the relay coil would go to the horn switch. The other side would go to constant 12v or switched with the ignition if you want.
One side of the relay contacts would go to ground and the other to the horn. And you can run a big (fused) power wire from the battery to your horn(s).
Or you could rewire the horn to have constant ground and switched power like in Mickey_D's drawing.
