It's worse when cold. Sometimes when it's been running for a while it gets to the point when it runs normal. After cold startups it's always really bad. Some days are worse than others. When the gas tank is almost empty, it gets noticeably worse.

Do fuel injectors ever just go bad, or do they only get dirty?
Definitely still sounds like fuel issues to me. I've never heard of a Toyota injector 'going bad'. They just get dirty, or gummed up when sitting around for a long time.

Have you cleaned your throttle body/intake manifold and injectors? Lots of work, but you'll be suprised how much it helps. Your cold start injector might be really dirty too.

As takza said, check the fuel pump too.
