I have an 01 corolla outside that started doing that when I was younger aND before I knew much about cars. My mother took it to a shop and all they did was run a positive lead to the relay and it's still there to this day. I looked into it a little bit but I haven't attempted a fix yet.

In the fuse box where the relay sits, I'm assuming it is not making good contact where it plugs into. I took the lead off and let the car warm up while wiggling the relay and it kicked the fan off on an, so my problem is inside the fuse box. You may want to try running a lead to the relay location and see if that kicks the fan on?

I bet the relay has gone bad.
Former Tercel Enthusiast (not a practical family car anymore but they still have a place in my heart)
